A Phoenix physical therapy clinic with 4 PTs was 92% insurance-driven. Insurance reimbursements averaged $42 per session. The owner had been told for years that "cash pay is a different world." It is — and that world pays $185 per session for the same work. The clinic just needed to be findable by people willing to pay it.
The challenge
The owner came in after a referral from a chiropractor we work with. The bot scored the website at 71 — fine for insurance patients, but nothing about the website spoke to a cash-pay buyer. Cash-pay patients (post-op, athletes, chronic-pain self-payers) want fast access, longer sessions, and 1:1 attention. They will pay 4× insurance rates for it.

The fix was to keep the insurance pipeline running (steady volume) but build a separate cash-pay funnel that did not undercut it.
The plan
Built /direct-pay-pt as the cash-pay front door. Hero copy: "60-minute 1:1 sessions with your PT. No insurance, no phone tag, no co-pay surprises. $185/session." The page targeted three personas: post-op recovery, weekend warrior with a recurring injury, and chronic-pain patients tired of insurance gatekeeping. Conversion rate hit 4.8% in 4 weeks.
Created a separate ad account/campaign for cash-pay terms only: "direct pay PT," "cash pay physical therapy," "physical therapist no insurance," "concierge PT." CPC averaged $4.80 — way below insurance-PT keywords because there was less competition. Conversion-to-booking on cash-pay ad clicks was 6.2%.
Cash-pay customers expect fast access. Built a booking page that showed real-time availability, with same-day or next-day appointment slots prominently surfaced. Card-on-file required to lock the slot. SMS confirms + 4-hour reminder. No-show rate was 3% on cash-pay vs 22% on insurance flow.
Wrote 9 long-form articles targeting search intent: "Why I switched from insurance PT to cash pay," "Post-ACL surgery cash-pay PT timeline," "Best PT for runners in Phoenix," etc. By month 3, those pages were ranking page-1 for 14 cash-pay-intent queries and producing organic traffic the clinic had never captured before.
